Account name: KSutcliffe

Sort code: 07 04 36    Acc no. 00501033 .

Your reference: Please leave your name or initials paying online, for me to identify who the payment is from.

I will also accept cash on the day.

CANCELLATION POLICY

Regular weekly attendance is important for the process of therapy and, by starting therapy, you are agreeing to commit to attending and paying for a session each week.

If, for any reason, you are unable to attend your appointment the full fee will be incurred. I will always try to rearrange a missed session but this will depend on my own availability and the availability of the therapy room.

However, if your holiday falls outside of mine there is no fee to pay, as long as 4 weeks notice is given. I cannot keep your session time open for you in the event of you taking a holiday longer than 2 weeks, unless payment is received.

I will endeavour to give you as much advance notice as possible in terms of holidays or of my need to cancel a future session.

CONFIDENTIALITY

I will keep confidential all that is said in our sessions. However, there is a requirement to keep notes, and discuss work in  supervision. In doing this, your identity will be protected by not using any identifying features (e.g. by changing your name/occupation etc). The purpose of consulting with others is to try to ensure the highest standards of professionalism and care in the work. Psychotherapists at times are required to make presentations, write papers and case studies. In all of these areas your identity will be protected. Psychotherapists, along with all Health Care Professionals, have a legal duty to breach confidentiality in certain circumstances when there is serious risk to the client or another individual.

ENDINGS

If you decide you no longer wish to continue with our sessions, conducting an ending is an important part of the process. I would urge you that rather than ending abruptly, we take some time to reflect on it.

DATA PROTECTION

I appreciate that you will be trusting me with sensitive and personal information, and I take your privacy seriously.

I take brief notes after each session which are kept securely. Notes kept do not identify you or people we discuss, they are for me to keep a track of the number of sessions and content that may be useful for our work. They are not official psychological assessments or reports. All personal data is password protected. Notes are kept for two years and then destroyed.

EXCEPTIONAL CIRCUMSTANCES

In case of emergency I am ethically required to appoint an appropriate colleague as my Professional Executor to inform you, and if necessary, provide support, should I not be able to provide therapy for a period of time. This executor is also bound by the same professional rules of confidentiality set out by UKCP and will only have access to your details if necessary.
